All primary care visit types included
Annual and routine physicals, back-to-school and sports physicals, preventive and wellness visits, routine appointments, chronic condition visits, hormone and joint visits, and all follow-ups. Medication checks and lab-result reviews are included.
No separate visit fee.
Same-day or next-day sick visits
Timely appointments for appropriate acute concerns, with care guided by your needs.
Visits up to 60 minutes
Time to ask questions, talk through concerns, and understand your next steps.
After-hours access
A way to reach your care team outside regular office hours for questions and guidance.
After-hours access follows the practice’s written policy. It is not emergency care. In an emergency, call 911.
Telehealth visits
Telehealth visits and virtual follow-ups are included with no separate visit fee, when clinically appropriate.
Prescription management and follow-through
Help managing ongoing prescriptions, communicating about refills, and keeping your care plan on track.
Medical weight management
Individualized medical weight-loss care and follow-up are included in the planned membership.
Medication and lab costs for weight management are not automatically included.

Physicals and preventive visits
Annual and routine physicals, back-to-school physicals, sports physicals, wellness visits, and preventive health discussions.
Routine care and chronic condition visits
Routine appointments, ongoing health concerns, chronic condition management, and women’s and men’s primary care.
Medication refills and management
Prescription refill visits, medication checks, treatment reviews, and follow-up for ongoing prescriptions when clinically appropriate.
Sick visits and follow-up appointments
Same-day or next-day sick visits, visits for new symptoms, follow-ups after illness or treatment, and lab-result reviews.
Telehealth and virtual follow-ups
Virtual sick visits, routine follow-ups, medication reviews, and results discussions when appropriate for remote care.
Hormone health and replacement therapy
Visits for hormone-related concerns, evaluation for treatment, and ongoing hormone-management follow-ups.
Medical weight management
Initial weight-management visits, individualized treatment planning, progress checks, and ongoing follow-up.
Joint and musculoskeletal care
Visits for joint concerns, treatment follow-ups, and select in-office procedures, including joint injections when appropriate.
Membership: All listed visit types, including hormone, joint, and telehealth visits, have no separate visit fee. Medications, labs, and procedures may carry separate costs. Future services: occupational health.
