Whether you’re looking for a primary care home or need a single appointment, The Haven is designed to make the next step easier to understand. Explore our monthly memberships and individual cash-pay visits below.
Primary care centered around an ongoing relationship, more time to talk, and a team that knows you.
Monthly primary care membership for one adult.
Combined monthly membership for two adults.
Added to an adult or couple membership.
Final membership terms control. Included visits do not mean every medication, test, injection, or procedure is included.
A primary care membership is not health insurance.
You can still request individual services at The Haven. Nonmember visits are billed per visit and are subject to appointment availability. Membership-only access benefits are not included.
Prices are for the visit itself. Medications, labs, and procedures may cost extra.
A visit may be included in membership while a medication, lab test, imaging study, outside service, or specialized procedure has an additional cost. We will tell you the price before treatment.
Cash-pay appointments do not include membership-only benefits such as direct provider access, after-hours access, or prioritized same-day or next-day appointments.
No. The Haven plans to offer individual cash-pay visits as well as monthly primary care memberships. Nonmember appointments are subject to scheduling availability.
No. A primary care membership is not health insurance and does not cover services outside the signed membership agreement, such as hospital, emergency, or specialist care. The Haven does not bill health insurance for the services covered by its membership agreement.
The planned membership includes many primary care visit types without a separate visit fee, including routine and sick visits, preventive and follow-up visits, and appropriate telehealth. Exact inclusions are defined by the final membership agreement.
Some services or products may carry separate charges. Ask for the price before treatment. Weight-management care does not necessarily include medication or lab costs.
After-hours access is a planned membership benefit provided according to the practice’s written policy. It is not immediate emergency care. For a medical emergency, call 911.
The practice plans to provide same-day or next-day access for clinically appropriate acute needs under its membership model, following its written scheduling policy and availability.
